HOW DO I SCHEDULE TIME AT THE SPORTS CENTER?
1. Contact the Field Rental Contact, Brian Connell, at bconnell@blueash.com
2. Turn in your signed Facility Use Agreement & COVID-19 Waiver. These agreements covers the usage of the sports fields that are located at the Blue Ash Sports Center (11540 Grooms Road). There is no deadline for this form, however, the Sports Center Supervisor must have already been notified prior to submitting this form.
3. Provide copy of team/organization insurance stating that the City of Blue Ash is additionally insured.
